David Baker
Meeting the Healthcare Reform Challenge
David Baker was named the CEO of Banner Home Care and Hospice in June of 2003. Prior to coming to Banner Health, Baker served as the regional executive director at St. John Home Care and Genesys Home Care in Detroit, Mich. he also held a position as the manager of consulting for Ernst & Young in Chicago, Ill. Prior to that, he served as the corporate director of Home Care Services, OSF Saint Francis, Inc., (a subsidiary corporation of a multi-state hospital system). Baker's experience also includes being a founder of St. Mary's Medical Center Home Care Division in Tennessee. He received his undergraduate degree from Lincoln Memorial University, and his masters of social work from the University of Tennessee.

Denise M. Blommel is an attorney in solo private practice in Scottsdale, Arizona. She represents employers and employees in employment and labor matters. Denise also is a contract mediator for the United States Postal Service and belongs to the panel of employment mediators for the American Arbitration Association. She is a member of the State Bar of Arizona, State Bar of Nevada, Community Associations Institute – Central Arizona Chapter, Scottsdale Area Chamber of Commerce, Scottsdale Bar Association, Arizona Employment Lawyers Association, Association for Conflict Resolution and Arizona Association for Conflict Resolution, where she is a member of the Board of Directors, and the St. Thomas More Society. Denise is a frequent presenter for government, business, educational and community groups.

Kathy McCanna is the Program Manager for the Office of Medical Facilities Licensing with the Arizona Department of Health Services, the entity responsible for the state licensing and Medicare certification of Arizona Hospices as well as Home Health, Hospitals, Surgery Centers, Dialysis and Outpatient Treatment Centers. She has over thirty years of experience in health care operations, quality and regulatory compliance. She earned a Bachelor Degree in Nursing and has certification in healthcare quality and as an investigator-inspector.

Correlate information at start of care to determine the plan of care, length of time until closure, and when changes to the plan of care will be appropriate. Joyce Norman RN, BSN, ET, CWOCN, and DAPWCA received her AD in nursing in 1974 and her bachelor’s of science degree in nursing in 1986. She attended Tucson Medical Center’s ETNEP, graduating in 1984 and has been board certified since 1985. Joyce has practiced in many health care arenas. She joined Medline Industries, Inc. after working 25 years in the acute care setting. She has consulted for home care agencies for 12 years, and has practiced the full scope of ET nursing for 23 years. She has taught and lectured extensively for the past 24 years.
